How To Get Enrolled
Every soldier in the ARNG, Officer and Enlisted, has the opportunity, and is encouraged, to apply for
active duty and correspondence schools. Attendance and successful completion of some military schools
are mandatory for promotion and retention. The following is information to give you insight and guidance
on enrolling for these courses.
- The Steps for Enrollment
- Always go through your unit of assignment. Enrollment request forms must go through your
respective Chain of Command for approval, prior to reaching the Office of the Adjutant General,
Schools Branch. Your Training/Readiness NCO or Unit Clerk has the responsibility for assisting
with your application request, and submitting it for approvals and enrollment.
- If you apply for enrollment in an AD school, i.e., AMEDD Officer Basic or Advanced Course,
use the Academy of Health Sciences Circular 350-3 for course dates and locations, and your
training NCO should be able to provide you with that information.
- If you apply for AD other than AMEDD courses, use Army Formal Schools Catalog, DA
Pamphlet 351-4, and your training NCO should be able to provide you with that information.
- The military offers many correspondence courses, including the Combat Lifesaver Course.

- Application forms and their use.
The application forms used in applying for AD and for correspondence courses differ.

- ARMY CORRESPONDENCE COURSES: DA Form 145, Army Correspondence Course Enrollment Application.
To simplify the application process, learn all you can about the course or school and its enrollment
procedures. Meet with your Unit Clerk or Training NCO to initiate your application. If your unit has
questions about an AMEDD school or course, they will use their chain of command to obtain answers.
You should assume the responsibility of periodically checking with your unit to get an update on the
status of your application. They can contact their chain of command for current information. The
Medical Recruiting Office can assist you and your unit/ chain of command with course or school
information.
